As an organization implementing a quality management system (QMS) based on ISO 9001: 2008, the ICT Unit of UB undergo periodic evaluation of the implementation of QMS ISO 9001: 2008 by an external party, namely, Lloyd’s Register Quality Assurance (LRQA) ISO certification agency. Certification of Quality Management System (QMS) ISO 9001: 2008 has been done for all working units in UB.

The process of QMS ISO 9001: 2008 certification for all work units in UB begins with socialization to all work units. Socialization is held gradualyl and tiered, for UKPA and UKPPA, as well as for officials, MR and the head of divisions. Certification of QMS ISO 9001: 2008 in UB uses the service of the same ISO certification agency, the Lloyd’s Register Quality Assurance (LRQA), so that the certificate number is same as that obtained in PJM, namely JKT6003728. Based on agreement between UB and LRQA , the certification of QMS ISO 9001:2008 is the extension of certification in PJM. To facilitate the QMS ISO 9001: 2008 certification, Certification Team was formed, consisting of the Rector, Vice Rector, expert staff, PJM, officials and MR of each working unit in UB.
First stage assessment of QMS ISO 9001: 2008 was held on December 27 and June 28, 2011, then the second stage assessment carried out on 11 to 14 and 17 to 19 October 2011. Working unit that became the object of sampling audit consists of 5 UKPA (FEB, FP, FT , FK, FMIPA) and 12 UKPPA (BAU, BAPSI, BAK, BAAK, LPPM, LP3, LSIH, ICT, SPI, PJM, JPC, Polyclinic). In the first stage assessment, there were 5 people of LRQA assessors involved, 1 person from AUT and 1 expert. First stage and second stage assessment involved 6 LRQA assessors and 1 person from AUT.

First surveillance of QMS ISO 9001: 2008

For three days (9 to May 11, 2012) LRQA assessor team ​​visited 19 working units (7 faculties and 12 support working units) and produced findings or records improve UB services. Based on the results of the 1st surveillance of QMS ISO 9001: 2008, Brawijaya University got recommendations from LRQA assessor team to maintain the ISO 9001: 2008 No.JKT6003728 certificate.

Based on the 1st surveillance QMS ISO 9001: 2008 results, ICT unit received a “Potential Major NC” finding related to the quality objectives which aim to measure the service level agreement (SLA) of internet connection established, but the summary report is not ready although the raw data has already available. Completion of these findings will be evaluated on the 2nd surveillance of QMS ISO 9001: 2008. Details for ICT Unit findings are as follows:

Second surveillance of QMS ISO 9001: 2008

Second surveillance of QMS ISO 9001: 2008 was held on 11 to 19 October 2011. Based on the results of the 1st surveillance QMS ISO 9001: 2008 on ICT units, there were findings in the level of “Potential Major NC” for quality objectives. In the 2nd surveillance of QMS ISO 9001: 2008, improvement has been done and findings became closed. In the 2nd surveillance QMS ISO 9001: 2008, the auditor found some minor findings such as:

  1. Customer satisfaction survey has been done but a conclusion and analysis of the survey results is necessary.
  2. E-complaint is already implemented filtering system in the e-complaint system should be considered. The follow-up of complaint handling from units or faculties can be applied by using softcopy.
  3. Consideration should be given to monitoring and recording for the temperature of the server room.
  4. Consideration should be given to keep the back up of data in another building for safety and security reasons.
